2. Service Description
Our NEMT service provides safe and reliable transportation for individuals who require assistance traveling to and from medical appointments, non-urgent medical facilities, and other healthcare-related destinations.
2.1. Eligibility
The service is available to individuals who:
•Are ambulatory or non-ambulatory.
•Require wheelchair or stretcher transportation.
•Do not require emergency medical care en route.
3. Reservations and Scheduling
3.1 Booking Requirements
•Reservations should be made at least 24-48 hours in advance to guarantee service availability.
•Same-day bookings are subject to availability and may incur additional fees.
3.2 Information Required
When booking, the following information must be provided:
•Passenger’s full name, contact information, and date of birth.
•Pickup and drop-off locations.
•Appointment time (if applicable).
•Type of assistance required (e.g., wheelchair, stretcher, etc.).
•Special needs or medical conditions.
3.3 Cancellations and Rescheduling
•Cancellations must be made at least 24 hours before the scheduled pickup time.
•Late cancellations or no-shows may incur a cancellation fee.
•Rescheduling requests are subject to availability.
4. Payment and Fees
4.1 Rates
•The cost of services will be based on mileage, type of transport required, and additional services (e.g., specialized equipment or extra assistance).
•A full breakdown of costs will be provided at the time of booking.
4.2 Payment Terms
•Payment is due upon completion of the service unless otherwise arranged.
•Accepted payment methods include cash, credit/debit cards, and approved insurance or Medicaid.
4.3 Insurance
•If covered by insurance, Medicaid, or another third-party payer, it is the responsibility of the passenger or their caregiver to ensure that prior authorization is obtained.
5. Passenger Responsibilities
5.1 Passenger Behavior
Passengers are expected to:
•Behave respectfully and courteously toward drivers and staff.
•Comply with any safety instructions provided by the driver.
•Refrain from any illegal activities or disruptive behavior during transport.
5.2 Medical Conditions
Passengers or their caregivers must inform the company of any relevant medical conditions, mobility restrictions, or special needs when booking the service.
6. Company Responsibilities
6.1 Safe Transportation
We commit to providing a safe and timely transport service, with drivers who are trained in assisting passengers with special needs.
6.2 Vehicle Safety
All vehicles are equipped with the necessary equipment for wheelchair or stretcher transportation and are regularly inspected to meet safety standards.
7. Limitations of Liability
•The company is not liable for any delays caused by traffic, weather, road closures, or other factors beyond its control.
•The company is not responsible for any medical emergencies that arise during transport. Passengers who require emergency medical care must arrange for ambulance services.
8. Privacy Policy
We are committed to protecting the privacy and confidentiality of our passengers. Personal information provided during the booking process will only be used for service delivery and will not be shared with third parties without consent.
9. Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of the state in which the company operates.
10. Modifications to Terms
The company reserves the right to modify these Terms and Conditions at any time. Passengers will be notified of any changes prior to their next booking.
